At Horizons of Hope, Inc. we take care to provide our customers high quality services personalized for their unique needs. Even though our administrative hours are 8:00 AM - 5:00 PM, we do provide services 24/7. Our staff members are professional, courteous, and efficient.
Community Inclusion: Personal Assistance & Transportation
Community Inclusion: Personal Assistance & Transportation
Personal assistance services are provided within the individual's home and/or community with activities. Our support services promote the individual's full participation in their community as well as opportunities and experiences with a focus on socialization and/or therapeutic recreational actives, personal growth, peer support activities, and organization participation in self-advocacy events.
Supported Living
Supported Living
Services are provided for up to 24 hours per day to enable an individual to live in a residence of the individual's choice with other individuals (within the perimeters allowed by DODD). All provided services encourage and support autonomy while promoting the individual's rights.
Homemaker - Personal Care
Homemaker - Personal Care
In order to meet the individual's daily vital needs, a variety of coordinated services, supports, and supervision is provided. These services provide opportunities to improve the individual's independence within their home and community.
HPC Transportation
HPC Transportation
Individuals on a waiver will receive services to access waiver and other community services, activities, and resources.
Non Medical Transportation - Per Mile
Non Medical Transportation - Per Mile
Waiver participants receive transportation services to access adult day support, vocational habilitation, supported employment enclave, and/or supported employment-community waiver services as allowed in the individual's service plan.
